Media Release - proof of vaccination

Media Release – September 22, 2021
Tweed, ON

 

Effective September 22, 2021, the Province of Ontario will require patrons to provide proof of being fully vaccinated along with identification showing name and birth date to access certain businesses and settings. Starting Wednesday, September 22, 2021, all persons entering the Tweed Public Library Meeting Room, Kiwanis Tweed Pavilion, Lions Hall, Actinolite Hall and Thomasburg Hall will need to provide proof of full vaccination, along with identification, unless they have an exemption. For all persons entering the Tweed Hungerford Community Centre and Andy Brown Memorial Centre, the following is required:

Vaccination Proof Not Required Exemptions
 

• Medical exemptions (with documented proof from a physician or by a registered nurse)
• Children under the age of 12
• Organized committee meetings

 

If you have questions about how to obtain a copy of your vaccination receipt, visit COVID-19 vaccination provincial portal or call the Provincial Vaccine Booking line at 1-833-943-3900. Proving Identity:
 

Validation of identification must also be undertaken to ensure that vaccination receipt offered by the patron belongs to them. Proof of identity can be established using documentation issued by an institution or public body, provided it includes the name of the holder and date of birth.
 

Examples of identification documents that may be used to confirm the identity of the holder of the vaccine receipt include:
 

• Birth certificate
• Citizenship card
• Driver’s licence
• Government (Ontario or other) issued identification card, including health card
• Indian Status Card /Indigenous Membership Card
• Passport
• Permanent Resident card

 

Validation of identification is considered to have been completed/successful when the name and date of birth of the presenter of the vaccination receipt and the name and date of birth on the identification document match. If the name and date of birth on both documents do not match, the individual will not be allowed to enter.
